How to Prepare for Your First Appointment

Before your first appointment take a few moments to:

1. Have your prior medical records and any previous films related to your condition (including MRI, CT scans, and x-rays) transferred to our office. You will need to sign a medical release form before your previous doctor can send your records. To download and complete forms, please see Patient Forms.

2. Secure a referral from your primary care physician if required by your insurance plan.

3. Determine if the Baylor College of Medicine orthopedic surgeon you have an appointment with is on your insurance plan.

4. Complete the patient history form(s) for your physician to help speed your registration process.

The day of your first appointment, arrive 30 minutes before your appointment. Bring your current insurance card and driver's license or picture identification. Medicaid/Medicare patients need to have a valid card and letter, as well as secondary insurance information. Be prepared to complete any additional paperwork.
