Thank you for your interest in referring a patient to our clinic. Our clinic does not require a physician referral but your patient’s insurance may require a referral for our service to be a covered benefit. There may also be times when the patient is not able to clearly specify the reason for the visit. In such cases, we may ask for a referral to better understand the patient’s needs. We see patients with private insurance, Medicaid, Medicare and patients without insurance on a “self-pay” basis.
Patients are seen at Baylor Medicine at McNair Campus, 7200 Cambridge Street, Suite 9B Houston, Texas 77030.
Steps for obtaining an appointment:
- Ask the patient to call our scheduling center at (713) 798-7820.
- If you wish to enter a referral, Baylor Medicine providers can refer to Adult Genetics in Baylor Medicine EPIC (REF26) or external providers can submit their referral online using this form.
Note: If you enter a referral in EPIC, your patient does not have to call for an appointment. They will be contacted by the scheduling center; however, they may also call for an appointment if they do not want to wait to be contacted.
- Your office may be contacted if medical records are required in order to schedule the appointment. We generally do not need all records but you or the patient will be informed if select records are needed to provide optimal care.
Records can be faxed to us at (713) 798-6450 (if less than 20 pages) or mailed to Adult Genetics at One Baylor Plaza, BCM228, Houston, TX 77030.
- We have a very limited number of overbook appointment slots but if you feel your patient needs to be seen sooner than the appointment given, please email adultgenetics@bcm.edu. We reserve these slots for truly urgent patients – for example, when the patient’s immediate medical management will be impacted. Requests for sooner appointments are reviewed on a case-by-case basis.
