How do I refer a patient for a BCM Undiagnosed Diseases Center evaluation?

Content

NOTICE: Pending regulatory guidance, we currently cannot accept residents outside of the United States as Consultagene account holders.

  • Create a provider account in Consultagene. Consultagene is a secure online platform being used by the BCM UDC team to interface with both patients and providers.
  • Log into Consultagene and complete a "Request for Referral". Please make sure that "Undiagnosed Diseases Evaluation" is selected for the "Clinic/Program" field. You will be asked to provide the following for the request:
    • Demographic information about the patient
    • Reason for referral
    • Optional: clinic notes and test results, which may be uploaded to the “attachments” section of the request

After you submit your request, the BCM UDC team will complete an initial review of the referral request. If your referral is determined to be eligible for additional evaluation by the BCM UDC, your patient will be invited to complete a patient “journey” within Consultagene.

Possible Outcomes

Content
  • There are no suggestions for additional testing because the patient’s previous evaluations have been thorough and appropriate.
  • Additional clinically indicated tests or consultations are suggested that your patient can complete through you or a local healthcare provider. Suggestions may also include enrollment in other research studies.
  • We offer your patient acceptance in our “Undiagnosed Diseases Evaluation,” which may include a combination of evaluations at home, via telehealth, or in-person at the Texas Medical Center in Houston, Texas.

We do not provide follow-up care; rather, we rely on you, the referring provider, to retain your role as your patient’s ongoing care provider. We look forward to working with you in your efforts to find a resolution to your patient’s diagnosis.
